Providence is calling a Full Time Emergency Department Outreach Specialist to Providence Milwaukie Hospital in Milwaukie, OR.
The Better Outcomes thru Bridges (BOB) Program Emergency Department (ED) Outreach Specialist works with a vulnerable population of people that have high emergency department utilization. The Outreach Specialist works in conjunction with patients, team members, peer support, hospital staff and community partners to develop and implement strategies that improve the health of individuals who are identified with high utilization of emergency department services. Patients with high emergency department utilization often lack access and/or knowledge of a connection to primary care or other health or community services as a means to address chronic or non-acute conditions. The ED Outreach Specialist is responsible for performing duties independently and as a part of the Outreach Specialist Team. The ED Outreach Specialist works out of the emergency department and in the community to coordinate care that helps bridge the gaps and eliminate barriers to health care and social services for this vulnerable population of people.
This particular role is funded through a Clackamas County grant and will work with houseless people who are allowed safe overnight car camping or placed in transitional and respite micro-homes located on various properties in unincorporated Clackamas County. This is a one year grant. This role will be responsible for assisting to lead community meetings, organizing and facilitating networking meetings with community partners, collaborating with the team specifically for this pilot project, including but not limited to, project manager, data analyst, various community congregations, community partners and others identified as key team members throughout the project. This is a 1 year grant funded position.
In this position, you will:
Work with a vulnerable population that has a high utilization of the Emergency Department.
Work in conjunction with patients, team members, hospital staff and community partners to develop and implement strategies that improve the health of individuals who are identified with high utilization of emergency department services.
Organize, collect, review and report health and social information through member home visits and phone outreach, while demonstrating multicultural sensitivity and effective communication skills.
Required qualifications for this position include:
American Heart Association Basic Life Support (AHA BLS) for Health Care Provider completion or obtained within 1 month of hire
Valid drivers license and reliable transportation
2 years of experience working with diverse vulnerable populations, community and/or health care organization experience
Knowledge of State social service agencies and community resources
Knowledge of health systems, health education, motivational strategies, and an empathetic manner working with the underserved
In compliance with company policy and state regulations, a Department of Human Services (DHS) criminal background check may be required for this position based upon the location of the position. Fingerprinting may be required also
Preferred qualifications for this position include:
Degree in social work, social sciences or human services field
Direct care experience in a residential and/or outreach position serving vulnerable populations
