Providence Health & Services Supervisor - Anticoagulation and Pharmacotherapy Services in Milwaukie, Oregon
Providence is calling a Supervisor- Anticoagulation and Pharmacotherapy Services (1.0 FTE, Days) to Providence Milwaukie Hospital in Milwaukie, OR.
Exciting new anticoagulation service pharmacist supervisor position that entails operations supervision of our PMG anticoagulation clinics and over site of our service clinical support specialists.
In this position, you will:
Provide operational and clinical leadership for Anticoagulation and Pharmacotherapy Services in the Portland and Yamhill service areas, and within Providence Medical Group (PMG).
Have the responsibility for coordination of standards and workflows across service locations, with responsibility for operational oversight of the extended locations of care beyond the hospital campuses.
Required qualifications for this position include:
Graduate from an accredited College of Pharmacy and , PGY-1 Residency training and 2 years of experience within relevant practice setting -or- , PGY-2 Residency -or- Without Residency a minimum of 4 years of experience including direct patient care experience in the area of anticoagulation dosing and monitoring
Current registration and licensure by the Oregon State Board of Pharmacy or eligibility upon hire
Board certified by the Board of Pharmacy Specialties in Pharmacotherapy, Ambulatory Care, and/or certification by the National Certification Board for Anticoagulation Providers required within 2 years of hire
2 years of experience and progressive responsibility in pharmacy management including Supervisory/Coordinator experience, demonstration of supervisory and administrative ability shown during that period
Preferred qualifications for this position include:
2 years of experience in ambulatory care
Demonstrated experience and success in operational process improvement
